Office Policies

- Please bring an I.D., your insurance card, and a list of medications to each visit.

- Please allow one hour for your appointment. We ask for your understanding when you are requested to wait beyond your appointment time to see the doctor. Surgical procedures may take longer than planned and emergencies do occur.

- For new patients, please plan to come 15 minutes ahead of your appointment time to fill out paperwork.

- If you make an appointment and are unable to keep if for any reason, please call us as far in advance as possible so that we can use the time for other patients. If necessary, we will be more than happy to reschedule your appointment as needed. While there is no cancellation fee, we ask for your respect for others who may want to receive services at that specific time.

- We take almost all insurances. For a list of accepted insurances, please click here.

- All doctors take the same insurance.

- If you have an HMO insurance, you must have a proper referral from your primary doctor.

- Copayments are due at the time of visit.

- Parents/guardians should accompany minors (children under the age of 18) to the office. An authorization consent form may be filled out and signed by the parent/guardian before treatment of a minor.

- When you come in, please advise the receptionist of any change in your insurance coverage, address and phone number that may have occurred. All information in your medical record is confidential and is released only by written authorization from you.

- All procedures are done in our offices.

- Patients will receive a reminder call 2 days before the scheduled appointment.

- Prescriptions and refills are issued during office hours only. To order a refill, please have your pharmacist call our office with your name, date of birth, and the purpose of the requested prescription.
